THere are various things about American F&B service that I find insulting, some of which because I'm European (I don't like being asked while I'm eating if evertyhign is good or if I need something; I'll get your attention of that's the case), but the worst offender is this.
Don't ask me if I want my check split; chances are pretty good that I'm treating the person(s) with me.
But don't get me wrong, in general the service is leaps and bounds better here than there. And a siggnificant reason for that is tipping culture.

What a nightmare. But, if you're a single 30/40 something man, and are so inclined, if you start a little banter with that table, your odds are 90 percent or better (closer to a 100 if it's a table of nurses)*
My minimum tip is 20 percent, even if the service is lousy, because it's my understanding that in most places the tips are pooled and so I figure why penalize the entire staff because I got a crap server. And getting a crap server is extremely rare in my experience. Most people that do that job try their best, especially if you set a good tone with them, right off the bat.
*(Girthang, I always cringe a tad, when I go a little misogynistic because I don't want to offend.)

I want to be clear about something. I'm not saying that teachers and nurses are more "morally ambiguous" than average, I just think those groups tend to go out together more than other professions, and that dealing with sick people and/or kids all day lends itself to blowing off steam.

If it's your experience, how is it misogynistic? Does that make my friend racist because he'd rather wait on people that aren't pigmented like him? I don't think it works that way. He said they're horrible tippers, so he avoids serving them.
I'm sure not all church folks tip poorly. My experience was that they didn't tip well on Sundays more often than others, so I chose different shifts.
I go out with the plan to tip a minimum of 20% and usually am between 25-30%. If the server is really bad of their own doing, they may get less than 20%, but that's rare for the reasons you state. Most of my family held jobs where they counted on tip income at some point in their lives, so it was offensive to me if someone was a shtty tipper and not on a fixed income. I didn't expect much from older folks on SS and was glad to see them and serve them. I knew they struggled just to afford the meal, when they had earned and paid in a lot more than they got back.
One of my step relatives was a horrible tipper, so I started paying my own way as soon as I was old enough to get a job if we went out for a meal.
