Hmm, I'm going to differ with you here, especially on the "critical thinker" side of your chart.
What's in it? I'll just do Pfizer:
mRNA - the only active ingredient
lipids -
- ((4-hydroxybutyl)azanediyl)bis(hexane-6,1-diyl)bis
- (2-hexyldecanoate), 2 [(polyethylene glycol)-2000]-N,N-ditetradecylacetamide
- 1,2-Distearoyl-snglycero-3- phosphocholine
- cholesterol
salts -
- potassium chloride
- monobasic potassium phosphate
- sodium chloride
- dibasic sodium phosphate dihydrate
sucrose
Who developed it? Thousands of researchers had their hand in developing mRNA technology over the past 30+ years. This one was developed by Pfizer/BioNtech
Who manufactured it? Pfizer and BioNtech
What is their track record? With what exactly? Pfizer was founded in 1849. They appear to be one of the better pharmaceutical companies.
Do they have legal liability? No, vaccine manufacturers don't have direct liability and for good reason.
How long was it tested for? Months, but it isn't as simple as that. A much larger trial was performed than normal to allow for more rapid use. The basic technology has been studied for decades. This is a biased question, not one of critical thinking. You should probably read this.

Who is telling me to get it? Both government entities and 99% of the medical profession. This is another biased question that has nothing to do with critical thinking.
What is their track record? Who exactly? The medical profession? Quite good. The government? Who cares.
Is it effective? Good question but too broad. How is effective being defined? Does it prevent the virus, yes, but not as much as hoped. Does it protect against serious illness and death? Yes, it does appear to be effective in that manner.
Is it necessary for my age? A good question and one to take up with your physician.
How many side effects reported? Terrible question. Reported means little. I imagine the question is referring to the VAERS system which is open to anyone to report anything, true or not. Confirmed side effects along with probabilities would be a better question.
How many deaths reported? See previous answer.
